Central Reach is your child’s clinic hub. This is where all medical records are stored as well as information about your child’s plan and the progress that has been happening during services. Messages within Central Reach are used for direct communication with providers as well as community updates to all of our clients. Please review your messages regularly to stay up to date!

Humana Military Patient Portal

Find your current referrals, authorizations, and message your provider. The Humana Military Beneficiary Self-service portal holds all of your primary care information.

Tricare-West for Beneficiaries

Make a payment, verify eligibility, PCM status and deductibles. Tricare-West portal can be a space to request PCM changes or even find your nearest military training facility. 

Deers

In order to qualify for benefits, all members must be active in DEERS. Manage benefits, view coverage, and make updates all in one space.
